Sorry, just figured out where it is.  

Found an additional error:

6) Some pages do not load on first attempt, required "Try Again" (again using 
a proxy).  e.g. https://mail.kde.org/mailman/listinfo/rekonq.  It appear to 
happen on first visiting any link at https://mail.kde.org/mailman/listinfo but 
revisiting the page is not that reliable at producing the problem.  Same thing 
happened when visiting https://mail.google.com/
