D9974: Improve header, metadata display, and version chooser UI

REVISION SUMMARY
  - Moved the icon back onto the page, and removed it from the header
  - Move app metadata below the description, to let users focus on the icon, screenshots, and description
  - Added blue lines below each header to better separate them from the content
  - Joined the version number and source together to communicate their relationship when more then one source is present
  - Implemented a combobox to switch versions instead of blue-colored link text and an overlay
