Oh, this would be a relief.  I like the above approach best since it would be transparent to older/simpler themes and provides additional flexibility to the theme authors.

Maybe having a panel-horizontal.svg, a panel-vertical.svg (or maybe even a 
panel-bottom.svg, panel-top.svg, panel-right.svg and panel-left.svg (1)) and 
falling back to panel.svg?

(1) Four svgs because lighting / shadows might be incorrect if you just mirror 
some svg
