This bug affects me too but not all pdfs with forms seem to be affected.

Kubuntu 26.04
Okular Version 25.12.3
Kde Plasma Version 6.6.6

Steps to reproduce:
Download pdf, for example:
https://buergerservice.bietigheim-bissingen.de/fileadmin/user_upload_stadtverwaltung/seitenstruktur/buergerservice-rathaus-politik/stadtverwaltung/buergeramt/Wohnungsgeberbest%C3%A4tigung_02.pdf
Click "display forms"
Enter or paste "AAAÄÄÄOOOÖÖÖ" in a form
Click "display forms" again
-> Umlauts are either not displayed at all or a placeholder is shown.
