Dutch Tax Office PDF form with some date fields not recognised by Okular

SUMMARY
Okular does not recognize the date input fields in section 2a of the attached
PDF as interactive form fields, while other viewers like Ubuntu Gnome Document
Viewer version 48 do.

STEPS TO REPRODUCE
1. Open the attached PDF file (model_opgaaf_gegevens_loonheffingen vanaf
2024.pdf) in Okular.
2. When prompted, click "Show Forms" or activate form viewing.
3. Scroll to page 2, section 2a.

OBSERVED RESULT
The date fields next to "Ja, toepassen vanaf" and "Nee, niet (meer) toepassen
vanaf" are not identified as form fields. They are rendered as static text
boxes.

EXPECTED RESULT
The date fields should be interactive and allow the user to type in the day,
month, and year.
