PDF demonstrating the problem

Hi, 

when i enter values into the input fields of the form on the last page, and
then hide the form, the entered values are not displayed. Showing the form
again gives you the editable input fields with all the entered values again. 
For some reason the values are not displayed when the form is not shown.

STEPS TO REPRODUCE
1. Open attached PDF with okular and show form
2. Enter input fields on the last page
3. Hide the form 
4. all entered values are no longer displayed.
