For check-marks problem: open document, show forms, check box 1B "Normal
Distribution–Roth IRAs", then check box subordinate to that one, "This is a
Qualified Distribution".  Save document.  Open document again, look at those
two check-boxes.  Box 1B "Normal Distribution–Roth IRAs" will be un-checked.
