test document

The Arabic inline notes are not rendered correctly. It lacks the bidirectional
text support as well as the proper shaping (i.e. the letters are disconnected.)

In the attached test document, the same text which are in original PDF is put
as inline note. 
The text is: بسم الله الرحمن الرحيم
