I observe same bug on Fedora 34 x86_64, okular-21.04.3.

Bug caused by non unique id "okular_fax" in the KPlugin json configuration:
https://invent.kde.org/graphics/okular/-/blob/5447aa1021a2313c4e4cfddbd3a0abb86270ee13/generators/fictionbook/libokularGenerator_fb.json#L124.

Same id is used by the fax generator plugin.
