I just tested the latest stable version for Windows I could find - Version
1.11.70

The problem is the same. Probably not so much, or not at all with the icons
from the Okular itself but the contents of the pdfs are not how are they
supposed to.

I changed back to scaling 100% and the problem disappears - everything looks
normal.

So, yes, the problem is caused by having a scaling different than 100%.
