I rarely get heavily annotated documents, but what Nate describes seems like a
very plausible workflow to me.

Regarding Dileep's question in Comment 6: I think annotations should not be
allowed to be dragged outside of the viewport, even if they do scroll along
with the document.  Rationale: I could probably still get them stuck, for
example by scrolling to the top of the document, and moving the annotation
upwards until it is outside of the viewport.
