Confirmed with the current okular git master, and poppler 0.48 from Debian
Testing.

Also confirmed with Atril (the Mate viewer), so supposedly it is a poppler
issue.

Incidentally, just opening and closing the document in okular gives me console
error messages:

~/tmp> okular 2016-Form1f.pdf 
evaluate() threw an exception
[unrelated stuff]
LEAK: 94 KJS::Node

I don't know whether that is related, though.
