I finally set up my own VirtualBox image trying to reproduce the bug. I applied
your patch -- and I cannot reproduce the bug with the given testcase under the
VM. I tried to reproduce the crash with another test case which crashes okular
frequently under 10.4.

When I tried the very same file within the VM I could not reproduce the crash.

The difference seems to be this message:
QPixmap: It is not safe to use pixmaps outside the GUI thread

If it appears (without patch), I can crash okular frequently. If it doesn't,
okular runs.

Therefore, I'm closing this bug. Thanks for your efforts.
