Forms are unavailable because that document is wrongly created and does not
state any form.

1. Open a wish for that
2. Well, you always need a default, no?
3. That's what the annotation editor in the settings is for, you want it to be
white by default, change it.

All in all i don't see the need for this bug, you should create a new one for
1) though.
