Sometimes the document gets reloaded, but all pages are black. Which is solved
after a second (automatic) reload.

It is possible that Okular is reloading the document too early, before the PDF
writer is finished outputting it? Resulting in crashes (due to
unexpected/missing content) or the black pages?
