The form at the above URL can be opened in Okular and edited (albeit without
saving form data).  Printing, either to preview or printer, however, fails
silently: nothing is displayed, either on preview screen or the (un)printed
page.

This has been seen on 2 separate machines running Kubuntu 12.04, with 2
different printers, one an HP LaserJet 4 Plus, one an unknown Epson inkjet
model.

Reproducible: Always

Steps to Reproduce:
1. Open PDF at above link.
2. Try to Print Preview.
3. Try to Print.
Actual Results:  
Blank screen/pages

Expected Results:  
PDF output

Since this is a PDF form file, I wondered if that could be the issue.  It
printed okay under Windows 7 using Adobe Reader 10.4.1, so the file doesn't
seem to be corrupted.  Thanks.
