some screenshots of my system (okular <-> acroread)

Okular saves the formular input data, even if you download a file again from
the web. Please delete:
~/.kde4/share/apps/okular/docdata/255107.CertificateStreet_BL_135(2).pdf.xml
(or similar)

The beaviour of my system is documented in the attached screenshots.
screenshot_1_orig.png: Initial load of your PDF
screenshot_2_form.png: Change the first input field to "Books Books Books"
screenshot_3_result.png: Hide the formular fields and you can see your input
embedded in the document

Which version of poppler do you use. I have 0.20.5 as it is marked stabel in
Gentoo.
I guess in Ubuntu it's 0.20.4.
Try a newer Version and file a bug at Ubuntu if this solves your problem
