Seems this is yet another XFA form with the same issue as
https://bugs.freedesktop.org/show_bug.cgi?id=55978. XFA forms are not supported
by Poppler yet.
Starting from Okular 0.16 + Poppler 0.22, you get a warning telling that this
document contains XFA forms, which are not supported.
