I had the problem for a while. 
During this time the fault could be re created every time I used Okular. 

The i found the reader mention in the synaptics package manager for Linux Mint
9.
I installed this reader (designed to work on okular)
Sorry i informed a wrong name it was named "Omaque"

I checked my note and have also installed "okular-extra-backends" the same day,
and since I haven't been able to re-create the problem.
