One reason seems to be a corrupted file ~/cups/lpoptions. After renaming this
file okular prints fine.

~/cups/lpoptions:

Default Photosmart_C7200 PageRegion PageSize=A4 sides=two-sided-long-edge
å=true
Dest Photosmart_C7200_fax PageSize=A4
