To #50 and others, I also looked at the KDE settings-manager
hardware->printer->your printer options while investigating. 

Have a look there at the 'print task options' tab of your the printer settings
and check what 'orientation' setting you have.

To #50  what if you have both landscape and portrait pages in the same pdf ?
