I found similar bug, but couldn't reopen it.
I installed KDE 4.4.0 on Windows XP x64 SP2, using MSVC 32bit packages.
In temporary folder there is file:
poppler-vc90-0.12.2-bin.tar.bz2
According to

I downloaded and extracted this file
http://winkde.org/pub/kde/ports/win32/releases/stable/latest/openjpeg-1.3-1-bin.tar.bz2
but this lead to crash of okular after opening any PDF file (unhandled win32
exeption), debugger shows access voilation in malloc.c).
I suppose it is installation bug, and I have manually downloaded incorrect
package.

Okular doesn't open PDF files in the KDE 4.3.1 and KDE 4.3.2 builds for
Microsoft Windows, when trying to open them on Windows 7 (Release Candidate
build 7100). Other files are fine (at least the JPGs I tested).
I uninstalled all the files, then downgraded to KDE 4.3.0, and now it works
fine.
I'd say the culprit is poppler 0.12.0, shipped in since 4.3.1 release. In the
0.12.1 release notes they say they fixed a lot of WIN32 issues, which might be
related to this problem.
