fwiw, on openSUSE 11.1, with

 okular -version
  Qt: 4.5.1
  KDE: 4.2.86 (KDE 4.2.86 (KDE 4.3 >= 20090514)) "release 126"
  Okular: 0.8.80

 kde4-okular-4.2.85-96.1
 libqt4-4.5.1-44.1

it seems to be 'behaving itself' ... last several printouts did NOT require A4,
correctly using specififed 'Letter' size paper.
