PDF showing the problem described.

To confirm bug, open pdf in Okular.  The text in the form field will be
rendered correctly.  Then hit "Show Forms" and put the cursor in the form field
  You don't need to actually make any changes.  Then hit "Hide Forms."  The
text now appears oriented at the right side of the form field, rotated 90
degrees to the right.
