under Bug 197693 -- will get back to you as soon as I can.

As for (3), if the form fields are not activated, you can see Japanese
characters at those spots:

Once fields are activated, some big input boxes cover the Japanese, see here:

If things worked alright, these boxes should be transparent so that you can
still see the Japanese text underneath. Clicking into the boxes would add a
check mark at the respective place, which currently doesn't seem to work.

Since everything else is Poppler related, I would assume this one is, too, so
I'll wait until I can upgrade.
