While I don't care about Print button being shown by default, I can't configure
toolbars to include the button where I want.

On Configure toolbars dialog, I can customize 2 toolbars: okular_shell (empty
by default) and okular_part. Print button is only avaiable on okular_shell. I
can't put okular_shell icons on the right of okular_part, forcing me to have a
print button as the first icon.
