As Albert pointed out, okular is showing whatever the document contains as its 
"title", not the filename. That could be anything - hopefully it is a 
representative name, but it could be blank, or "NONE" or something misleading.

You might like to look at the metadata (using File->Properties in okular, or 
using a command line tool like pdfinfo) for your file to see this.
