I have started to work on a quick and dirty program which reads the okular xml file containing annotations for a specific pdf file and uses podofo to add these annotations as (real) pdf annotations to the file. Right now it only handles text annotations (i.e. okular type 1) and even that not perfectly (i.e. it doesn't preserve creation time, author etc). If you think this is not a complete waste of time, I can improve it and put it up somewhere for download.
