Waiting for more elegant solutions, okular might store annotations inside documents using something like:
"pdftk doc.pdf attach_files annotations.xml output doc.pdf" for pdf
"djvused doc.djvu -e 'set-ant annotations.xml'" for djvu
"wrjpeg -cfile annotation.xml doc.jpg > doc.jpg"" for jpeg and so on...
Where this is not doable okular might fallback to the present solution.
