As for #4, Okular should use the go-next/go-previous icons at the bottom of the main view.

As for #5, I'm really not sure about what to do about this... if we introduce a third (!) set of arrow icons, how would they look, should they be naming spec compatible with the current ones (per fallback) and where to draw the line between using the one of the other? I throwed this into the naming spec discussion long ago, but it is yet to be determined if distinguishing icons in that way is a good thing or just confusing for users and developers.
