An overlay typically works by drawing the window in a solid color in VRAM and 
letting the video card replace that color by the video. However, most video 
cards support only one overlay at a time. So if another application has 
claimed the overlay, it could prevent kdetv from getting it.

Look for another application that deals with video, such as Kaffeine. Close 
it, or at least make sure it's not playing/paused.

If that doesn't work, you could configure kdetv not to use an overlay, but 
that will probably cause it to use more CPU cycles and become more sensitive 
to background processes that claim the CPU from time to time.
