Applying Jasem's suggestions for the AddCatDialog:

+ The window is now called "Import Catalog" rather than "Add Catalog"
+ Added preview box for imported file
+ Re-ordered widgets according to Jasem's suggestion

TODO: Add Pablo's buttons, and also ability to use arrow keys to 
manipulate the data-field items.
