OK, after a bit more thought here is what I'd suggest. Please check the 2
mockups I'm going to attach and comment. 
The code to write to get that dialog to work correctly requires some thought,
which is why the mockups aren't quite right in terms of what widgets are
enabled or not. But it seems to cover all reasonable cases I can think of with
a limited complexity.
