CVS commit by staikos: 

- remove File->New because it's confusing.  Maybe we'll add an alternative
  later
- cleanup kstdebug.cpp (getXXX -> xxx), remove unnecessary QString copies
       - return const& instead of pointer to member
       - note about internationalization problem
- remove unused rc entry
