Please go check under /kolab/var/imapd/spool/user/ folder and check if
the user account exists.

What might have happenned is that the kolab daemon has crashed or for
some reason is experiencing difficulties in creating the new mailbox. You
can also check /var/log/messages for clues.

Also try to run /kolab/etc/kolab/kolab -v -o and check the output for clues.
