when writing IPTC to the file, the md5 sum should be recalculated and
updated in the DB. Writing the IPTC (most probably) changed it,. so we
know, it should be updated.
(I had only a short look at the patch and couldn't see a direct
reference to something doing this. I'm sorry, if I simply missed it and
are asking for something already present)
