Synchronization for real is way complex and most likely not needed at
all (and most likely you did not talk about it).  Keeping this
essentially one-way (db to IPTC and IPTC to db for selected images,
overwrite or merge tags; was this what you ment?) simplifies a lot and
allows, I think, pretty much all that is needed.  

Everything but concurrent conflicting edits would go just fine if only
user knows what she's doing (ie how the concurrent edits should be
merged).  If the user can not be trusted to know what she's doing, this
too becomes complicated and either db to IPTC or IPTC to db must be
chosen and the other way prohibited to keep things simple.
