Calendar, the Kolab calendar is not displayed in the list of calendars
available when creating a new event so it is impossible to store events in it
atm.

Reproducible: Always

Steps to Reproduce:
- Set up an Imap and Kolab resource
- Launch Kontact and go to KOrg
- Create two Ical calendars by right clicking on 'New Calendar' in the resource
and folder view
- Both the Kolab Calendar and the Ical calendars you've created should be
displayed in the resource and folder view.
- Click on 'New Event' button
- Click on the 'Calendar:' combo box

Actual Results:  
The calendar combo box only displays:
* both Ical calendars: one available and one greyed if that Ical file is set as
a read only calendar 
* two Ical calendars and a Kolab (NameOfTheKolabCalendar) / My Data entry which
is not even a real calendar if no calendar has been set has read-only.

Expected Results:  
The Kolab Calendar should always be available from the 'Calendar:' combo box.

This is a regression since Calendar was available from that combo box before.
