- Launch KOrg
- Go to Settings menu/Configure Toolbars...
- Select the 'Delete' action from available actions and put it in current
actions by clicking on the right hand side arrow.
- Click on 'Change Icon...' button
- Select a different icon for it and click on 'Ok'
- Click on 'Change Text...' button
- Type in 'Delete incidence' and click on 'Ok'
- You can see the new icon and new name in the 'Current actions' table.
- Click on 'Apply' and a 'Delete incidence' button should appear in the main
toolbar
- Now click on 'Defaults' button
- A 'Do you really want to reset all toolbars of this application to their
default? The changes will be applied immediately' message appears in a pop up
dialogue.
- Click on 'Reset'
- The button disappears from the toolbar but remains in the 'current actions'
(unless bug#234571 has been corrected) so click on 'Ok' and reopen the
Configure Toolbars dialogue.

You can see in the 'Available actions' table that the 'Delete incidence' action
is now called 'Delete' again but its icon has not been reset to the default
one.
