I can confirm this on kopete 0.50.1, although I don't have a way to reproduce it.

It happened while the account without icon was logging in from one pc to another, and while signing in back to the first pc, the icon "blinked" between the online icon and no icon, and then it was fixed.

So it seems there is a contact-state that has no icon attached.
