An update:

This doesn't appear to be an issue with konsole or KDE. It seems to be a fault
in the kubuntu upgrade from 11.04.to 11.10 (and then through to 12.04). I have
confirmed this by performing upgrades from 11.04 to 11.10 and 12.04 which
results in the issue described above. I don't know what had changed between
11.04 and 11.10 but it appears that some older packages have remained (for
example, I get update notifications by two kde applications).

I have also tested on a clean install of 12.04 and the issue did not occur.

So I guess this can be closed.
