Problem:
When manually setting the tab title, it won't stick, even if the tab title
format in the config is left blank.

Reproduce: 
Login to a remote machine(call it cgi) multiple time, then shell from that
machine to another machines(call them app1, app2, app3) - I'd like the tab to
say app1, app2, app2 etc.   The predefined values see the original machine, not
the final destination so remote host name isn't correct(it reverts to cgi).  If
clearing the 'format' tab the tab title is continually reverted back to 'Shell'
- or whatever the profile's name is.

Expected Behavior:
If a user manually sets the tab title that should be the final be all name of
that tab - and it should be persistent.
