I figured out how to reproduce it. 

middle mouse button on a tab closes that tab without any warning, unless there
is obviously a program running in that tab.  I was obviously MMB-pasting and
missing the tab contents (I have a Thinkpad X60 which only has a trackpoint,
which gives me a -3 penalty to DEX)
