Actually one part of the bug was fixed, see comment #25.  Repeating the steps outlined in the original report summary, I get the expected result (bold text) in KDE 3.5.9.

What isn't fixed are the more complex issues described by Lars.

Unfortunately there isn't a "HALFFIXED" status so I called it WONTFIX.
