OK - found the cause.
In Settings | Configure KMymoney | General | Filter - The option 'Do not show
reconciled transactions' was set. 
I don't remember accessing this, but maybe I accidentally triggered it while
looking for something else, frankly I didn't even realise this option existed
until just now.

I'll flag this as resolved and my apologies for wasting your time.
