In my case, the unmapped accounts may be a red-herring.  There are still
two mapped accounts at that bank.

So, I unmapped one of them and tried update all accounts.  First time it
worked - that is, it didn't crash but produced a 404 error (there is no
longer an ofx capability at the bank.)
Second time, though, there was a crash, so mine appears not to be
related to 'two accounts at the same bank.'
