memory leak in HTMLElement operator=

Hi, 

this is another from the trivial patches department, was fixed about a year 
ago, still not in your tree. 

Also implements "all" which is useful for IE specific ECMA bindings, fixes 
code formatting issues and doxygen errors. 

Please merge.
