I am not able to reproduce this bug on Plasma 6.2.5 with Dolphin 24.12.1 (on
Solus) or Dolphin 23.08.5 (Kubuntu 24.04)
I see a preview on a folder with mp4 files and for the file itself 
View - show previews - checked
If I uncheck that, they disappear as expected

To try to get more information about the issue on your system, can you run
Dolphin from terminal and reproduce the problem?

>From command line just run

dolphin

Then navigate to your mp4 folder
Then paste the output into a comment in this report. Thanks!
