It happens on all icons. I normally don't use anything but the default Breeze
theme, but installed a 3rd party theme to test it, and it did the same. Is
there a way to "clear cache" on plasma? Reset it factory settings? I'd do that
since I don't really have a bunch of customizations anyway.
