So, the indexing is going on as it should (you are indexing content and
'balooctl monitor' is showing you the progress)

Sounds like:

    Doubling up of search results, Bug 475919, might catch them unawares
    (but that is something that is not "out of the ordinary" where your search
    results contains files with the same name)

    Confusing "empty" results when searching from a particular directory (as
    opposed to "Your Files"). That's the Bug 474973

There were issues with openSUSE and latterly Fedora. The systems were using
BTRFS and there were patches to solve them:

for KF6 and here:
    https://invent.kde.org/frameworks/baloo/-/merge_requests/169
for KF5. These are now a while back 

What is your filesystem? I'd be particularly interested if it is BTRFS or a
FUSE filesystem. If BTRFS it would also be interesting if you are doing
snapshots (and Baloo might be indexing them).
